Care navigation: A better way to experience healthcare

Today's healthcare system can be complicated. The Board of Pensions, in partnership with Quantum Health, is taking a significant step to simplify the journey for those we serve. Starting April 1, 2022, members in all three Medical Plan options (PPO, EPO, and HDHP) will have access to care navigation as part of their medical benefits. Care navigation helps members and their covered family members receive the right care, at the right place, at the right time.

Why care navigation?

Provided by the Board of Pensions in partnership with Quantum Health, care navigation helps Medical Plan members and their families navigate their healthcare and make the most of their medical benefits. Care navigation enhances the value of medical benefits through the Board of Pensions.

As healthcare has grown increasingly complex, the Board of Pensions wants to help members continue to have successful health outcomes. That's why we're providing care navigation to PPO, EPO, and HDHP members. Through care navigation, you will have access to Quantum Health's highly trained Care Coordinators who serve as your personal guide, making it easier to navigate today's complicated healthcare system. Care navigation also helps you become a better-informed healthcare consumer so you can take control of your healthcare journey.

Added value to your medical coverage

Care navigation complements your medical coverage through the Board of Pensions — the medical coverage features that you rely on will continue to work seamlessly. You still have coverage through Highmark Blue Cross Blue Shield (BCBS) and access to the same national BCBS provider network, and Express Scripts continues to administer your prescription drug benefits. Now you also have enhanced support and resources available when you need to access care.

Starting April 1, 2022, if you are enrolled in the PPO, EPO, or HDHP, Quantum Health is your primary point of contact when you have questions about your medical or prescription drug benefits or need help with a healthcare issue.* New medical and prescription drug ID cards with the Quantum Health toll-free number and website will be mailed to you in March. You should destroy your old cards and begin using your new cards effective April 1.

*Members enrolled in Triple-S and GeoBlue should refer to their plan provisions for details about their benefits and continue to contact those plans as before. Similarly, because benefits provided by the Medicare Supplement Plan are coordinated between Medicare and Blue Cross Blue Shield, members enrolled in that plan will continue to contact Highmark Blue Cross Blue Shield with questions.  

Supporting you at every step of your journey

When you call Quantum Health, you'll speak directly with a Care Coordinator who can help and support you at every stage of your healthcare journey, whether you are 

  • seeking to maintain or improve your health;
  • newly diagnosed with a chronic health condition, such as diabetes or heart disease; or
  • dealing with a complex medical issue, such as a cancer diagnosis or high-risk pregnancy.

With care navigation, you have someone to call when you have a question about your medical benefits or if you are facing a decision about your healthcare.  

Advocating for your care 

Your Care Coordinator serves as your personal healthcare guide, getting to know you and your health and wellness needs, and working with your providers to ensure you receive high-quality, safe, and cost-effective care. Care Coordinators are highly trained nurses, clinicians, and benefits specialists who advocate for your care.  

Care Coordinators are dedicated to helping and supporting you on your healthcare journey, no matter where you are. They are well versed in the medical benefits provided through the Board of Pensions and can answer your questions. Care Coordinators are available to speak English, Korean, or Spanish to best serve our members.
