Diabetes management program

The diabetes management program provided through Teladoc Health combines the latest technology with expert coaching to help those living with diabetes manage their condition. The program is provided as part of all three medical options, at no additional cost.

The Board of Pensions partners with Teladoc Health to offer the diabetes management program to employees and their eligible family members enrolled in any of the three medical options.

When employees or their covered family members enroll in the diabetes management program, they receive the following at no cost:

  • A connected glucose meter. This meter automatically uploads blood glucose readings to the member’s private account — no Wi-Fi or smartphone required. The member receives a personalized message to help them make informed choices for their health. They can also view trends of past readings at any time.
  • Unlimited test strips. Members get as many strips and lancets as they need with no copays, deductibles, or coinsurance. When more strips are needed, the member can simply tap the meter to reorder.
  • Access to a health coach. Experienced coaches, all Certified Diabetes Educators, are available 24/7 to offer support and answer questions about blood glucose readings, nutrition, or lifestyle changes. Members also can schedule phone appointments or get expert advice by email or text message.

Diabetes is a health concern that millions of people in the United States face, according to the American Diabetes Association. This program will help your employees better manage their condition and their overall health.

Cost

There is no additional cost to employers for the diabetes management program through Teladoc Health. This benefit automatically is included when you offer the PPO, EPO, and/or the HDHP.
